On April 8, 2025, a Zoox robot taxi collided with a regular passenger car in Las Vegas, USA. In response to this accident, Zoox has submitted a recall report to the National Highway Traffic Safety Administration ( NHTSA ) to conduct a software recall of 270 robot taxis.

Zoox provided detailed information about the collision that led to the recall: 'This recall is voluntary. The recall report submitted to NHTSA relates to an accident between an unmanned Zoox robotaxi and a vehicle. No injuries were reported and both vehicles sustained minor damage. The accident occurred when the vehicle was rapidly approaching the lane in which the robotaxi was traveling. The Zoox robotaxi anticipated the vehicle moving forward, slowed down and steered to the right. However, the vehicle came to a stop on the shoulder, giving way to the Zoox robotaxi. The Zoox robotaxi applied sudden brakes but was unable to avoid the collision.'

When the incident occurred, Zoox took immediate action as part of its internal processes and vehicle operations. The company's remote operations team notified the Las Vegas Police Department, all robotaxi operations were suspended, and Zoox began an internal safety review and analysis. Zoox also immediately reported the incident to the NHTSA.

According to a Reuters report, the problem appears to occur when 'a Zoox vehicle enters a driveway at a vertical angle while traveling at or above 40 miles per hour.'

After analysis and rigorous testing, Zoox was able to identify the root cause of the accident and is rolling out a software update to all Zoox vehicles to fix the cause of the accident. The update has been rolled out to all Zoox vehicles in operation, including test vehicles, as well as Zoox robotaxis.
