In a 1988 case report of oral insemination, a 15-year-old woman from Lesotho , South Africa, visited a hospital complaining of severe abdominal pain. An examination revealed that she was nine months pregnant, and that her uterus was contracting regularly and that the fetus's heartbeat could be heard.
However, the woman never dreamed she was pregnant because although her uterus was normal, she did not have a vagina that connected to the outside of her body, but instead had only a shallow slit covered by skin. This rare condition, where the vagina is absent or closed, is called vaginal atresia and is thought to occur in 1 in 4,000 to 10,000 newborn girls.
As vaginal delivery was not possible, doctors performed an emergency Caesarean section and a healthy baby boy was born, weighing 2.8kg.

While the nurse was stitching up the woman after the birth, she said she was well aware that she didn't have a vagina and couldn't have normal sex, so she tried to use oral sex as a substitute when she had sex with her boyfriend 278 days, or about nine months, before the birth.
However, when the woman was performing fellatio on her boyfriend, her ex-boyfriend, who had witnessed the act, burst into the scene with a knife and a fight ensued. All three were taken to the hospital with traumatic injuries. The woman was particularly seriously injured, having been stabbed in the abdomen and having two holes in her stomach, but after surgery she made a rapid recovery and was discharged 10 days later. No contents were found in her stomach, but doctors washed it with saline after stitching it up as a precaution.
Based on the details of the stabbing incident and the results of the interview, the doctors concluded that the knife had penetrated the woman's internal organs, creating a hole in her digestive tract, allowing the semen she had swallowed to enter her uterus and cause her to become pregnant. The stomach is not an environment in which sperm can survive, but saliva is almost neutral and has low acidity, and the secretion of gastric acid can be suppressed due to nutritional deficiencies, so it is possible that the swallowed sperm had the ability to fertilize for a while.

What's more, doctors say that the woman was likely to have been ovulating for the first time or close to the first time when she was stung, because after that point, clotted blood would build up in the uterus and fallopian tubes, making it difficult for conception to occur.
As the woman had never experienced menstruation, she was confused by the changes in her body, and although the possibility of pregnancy crossed her mind due to comments from those around her, she could not believe that she was really pregnant. However, the woman's family and her boyfriend, i.e. the baby's father, readily accepted the situation, and livestock were traded between the two families as a sign of reconciliation, so there was no great upheaval in the environment surrounding the mother and child.
The woman later underwent surgery to remove her uterus when her son was two and a half years old, but the baby boy she brought to the hospital looked just like his father.
